This test detects the 'infected' c ses of tuberculosis by eliciting a hyp rsensitivity reaction in patients who are lready infected with tubercle bacilli. In th s test, more precisely the Mantoux t st, purified proteins isolated from tubercle b cilli called 'tuberculins' are injected intracutaneously on the pper surface of the forearm. Out of s veral tuberculin tests, the Mantoux is m st commonly used, since by and l rge its results are highly reliable. Th s test will have little value in pl ces where there is a high ncidence of tuberculosis, i.e., at such pl ces almost every person can be xpected to be infected with tubercle b cilli. However, it has value otherwise. Th se cases which are tuberculin negative, may not be s ffering from tuberculosis, and in such c ses, one should think of other p thology in a concerned patient. In bout 48-72 hours, following the injection of the t st, an allergic reaction occurs as a r ised, hard red spot at the s te of injection. The size of th s area should be carefully measured, and an rea of 10 mm, roughly about h lf an inch or more in d ameter, indicates that the reaction is p sitive. Only the in duration (raised rea) should be measured with regard to its w dth. The redness around the raised rea must be avoided while taking m asurements. Ideally, a trained and experienced t chnician, especially one who specifically deals in c rrying out tuberculin tests, is required. An in d ration of less than 7 mm is c nsidered negative, while in duration between 7-10 mm, may be c nsidered a borderline or a doubtful c se of tuberculin positive, and such c ses need further tests, for exclusion, or for stablishing a diagnosis of tuberculosis.

However, this test has its own l mitations. Although a positive reaction does ndicate that the person is infected w th tubercle bacilli, it does not ndicate whether the disease is active or n t. Further, a false-positive reaction may lso occur when the person has b en infected with some atypical type of b cilli of this group, which have a cl se resemblance to tubercle bacilli. Furthermore, s nce now the BCG vaccination is g ven to all the newborns, especially in h spitals, all such children will be str ngly positive for the tuberculin test. In a few sp cific cases, a false-negative tuberculin reaction may lso occur, when the patient is s ffering from some other disease at the s me time; or the patient is on s me drugs which are immunosuppressive in n ture. In a nutshell, this test may not h ve any value in detecting the h dden/ symptomless, especially active cases of t berculosis, when a large-scale screening program is c rried out; even though this test has its v lue in individual cases.
