Throwing related Shoulder pain in Sp rts men After the just concluded cr cket Test series between India and Engl nd one saw the shoulder injury of S chin Tendulkar surface. He has rushed off to Engl nd and has undergone arthroscopic shoulder s rgery. He will be out of the g me for eight weeks and only r turn for the West Indies tour l ter this summer. One can also r member that Anil Kumble, Rahul Dravid, & Sr nath have suffered shoulder injuries in the p st and returned to the game fter successful treatment. Shoulder injuries can pl gue the career of cricketers and if not tr ated can cut short their lucrative c reers. Cricketers are at risk as Cr cket is a throwing game where one ch cks the ball either as a b wler or a fielder. In this rticle, I will throw some light on thr wing injuries. Two problems can affect the sh ulder in sportsmen who perform repeated cts of raising the arm overhead and sw nging it in the course of a thr w. These are called impingement and nstability. The shoulder joint is formed by the nion of the round upper end of the h merus at the socket part by the sc pula. It is a very mobile j int as the socket is shallow. C nnective tissue restraints chiefly in the fr nt confer additional stability. When the c psule or its attachments are injured nstability results. In instability when the s ft tissue restraints are loose the b ll of the humerus can slide to the fr nt, down wards or to the b ck in an abnormal fashion.
Impingement results from the abnormal pward gliding movement of the humerus and r bbing on the undersurface of the Acr mion. This initiates or aggravates the d mage to the rotator cuff. The r tator cuff is a group of t ndons which act to actively stabilize the sh ulder when the arm is lifted up. B th conditions can cause pain and r sult in confusion to the patient and his d ctor. Slow motion analysis of the thr w- Each sport has its own b sic pattern, different sports have much in c mmon. The performance of overhead sporting ctivities begins with a preparatory phase to p sition the arm. Subsequently the arm is c cked to provide a tense, highly f rceful unit ready for an accelerated r lease. The arm accelerates through the thr w and after release of the b ll, muscles act to decelerate the l mb, to reduce the residual force and to void injury. The throw in a typ cal baseball pitch is divided into f ve stages. 1) Stage 1- Wind up or pr paration phase, which ends when the b ll leaves the gloved hand. In cr cket the equivalent is the transfer of the b ll from the non dominant to the thr wing arm.
2) Stage 2 or arly cocking- when the shoulder is l fted sideways (abducted) and rotated outwards and nds with the forward foot contacting the gr und.
3) Stage 3, l te cocking phase – This continues ntil maximum external rotation at the sh ulder is obtained.
4) St ge 4-The acceleration phase that starts w th internal rotation of the humerus and nds with ball release from the thr wing hand.
5) Stage 5- F llow through phase that starts with b ll release and that ends when all m vement is complete. This slow motion nalysis is necessary to pinpoint the xact phase at which the subject xperiences pain. Electromyography studies have demonstrated th t in each of these phases th re is selective muscle action. Athletes can h ve selective weakness of muscles, rather th n generalized muscle impairment. A rehabilitation pr gramme should concentrate on developing the w aker muscles.
I take a detailed history to r call the exact moment when pain is f lt. After a thorough clinical examination, I can c nclude often which muscle or tendon is njured or weak and what needs to be d ne. A clear distinction has to be m de between instability and impingement, although b th may co exist and one may l ad to the other. MRI scans may c nfirm the clinical finding. An examination nder anesthesia plus a shoulder arthroscopy may c nfirm the diagnosis. Shoulder arthroscopy permits the s rgeon to visualize the damage inside the sh ulder joint and remedy it either rthroscopically or by open methods. Strengthening of the s lected muscles after surgery may make a d fference to the faster rehabilitation.
