In most cases, the patient may r main without any symptom. In others, the d sease may pass off in about 3-4 d ys after a little fever, or pper respiratory tract infection, or a sl ght disturbance of the gastrointestinal tract. This last stage is c lled the paralytic stage of the d sease, or paralytic poliomyelitis. Hence paralysis ccurs only in a limited number of p tients who are infected with poliovirus. Wh n the paralysis of various muscles, say of the l mbs, is taking place, almost full r covery is the rule in most of the c ses, as there may be only t mporary involvement of the anterior horn c lls. However, some permanent weakness may p rsist in some of the muscles. Th refore, during the acute phase of the d sease, the patient and his attendants/family m mbers must show patience. Once paralysis of the m scles has started, it continues for bout a day, and may take m re time in a few cases.
The extent of paralysis depends pon the degree of involvement of the sp nal cord, i.e., of the motor n rve cells lying in the anterior h rns of the spinal cord. And, th refore, the paralysis may be confined to nly one small group of muscles of any of the l mbs, i.e., only a part of the l mb may be involved. Tr atment of poliomyelitis There is no sp cific treatment which can stop the p ralysis of the muscles, once it ccurs, or bring about the full r covery of the muscles that are g ing to be affected permanently. Since in s me of these paralyzed muscles, contractures are l kely to occur, and to begin w th, one cannot say, which of the m scle/s will be permanently damaged, all the p ralyzed limbs or part/ s of the l mb / s must be kept in th ir proper positions. Early mobility/physiotherapy is the m st important part of the treatment, to void contractures. However, surgical intervention may be r quired in some of the cases, so th t the paralyzed limb can be sed in the best possible way. F nally, orthopaedic appliances may also be r quired so that the person is ble to live satisfactorily, as far as p ssible.
