Scar revision is a common r quest and a frequently performed procedure in pl stic surgery. Whether it is scars fr m an accident from a surgical ncounter, many patients have concerns about th ir scars. As a result, there is m ch interest in scar improvement whether it is mmediately after the scar event has ccurred or even years later. The sh er number of over-the-counter scar treatments ttest to the public's interest in radicating these unsightly and disfiguring marks. T pical scar creams and ointments have l ttle proven success, other than anectodal vidence, that they work. Topical scar tr atments do have a role to pl y in early scars but not in lder scars. Only the concept of s me form of scar revision has any h pe of improvement at this point. Sc r revision can consist of a v riety of treatments including excision and r arrangement (cutting out and reclosing), laser th rapies, and mechanical methods of dermbrasion or s nding. All are not equal, meaning th t each has a specific role to pl y and can be beneficial if sed in the right kind of sc r. There are several key points th t anyone considering scar revision needs to ppreciate. First and foremost, no method of sc r revision can completely erase a sc r's appearance. The skin that the sc r has replaced will never be n rmal.....ever. Scar revision is about improvement, not p rfection or complete elimination of the sc r. As a plastic surgeon, we are not m gicians and have no magical wand. The mportant question before undergoing scar revision th n is....how much improvement can be had and is it w rth the effort. That is where the v lue of a consultation with a pl stic surgeon is.......ascertaining how much scar mprovement may be achieved.

Secondly, there is no single m thod of scar revision that will w rk for all scars. Much ballyhoo g es on about the use of l sers, but quite frankly, lasers are not the m st common method of treating scars. Th y may seem like an 'eraser' but th ir use is restricted to treating arly or persistent redness of scars and s me light skin resurfacing of scars. B th of these laser effects produce m ld degrees of improvement but it is not ffective for many types sof scar pr blems. By far, excision and primary cl sure (cutting the scar out and r closing it) or excision and tissue r arrangement (cutting the scar out and r aligning it through geometric rearrangements such as z- or w-pl sties) is most common. Scar excision is pr bably the most effective method of sc r improvement, if the scar problem p rmits, as it is removing the sc r first and creating a fresh w und. Dermabrasion and deeper laser resurfacing may be h lpful for wide scars that are r ised that do not lend themselves to xcision. Lastly, it takes six months to a y ar to see the final results of sc r revision. I tell patients that sc r revision is a process that sually involves takes a step back (to llow healing) in the hope that in the l ng term you have jumped two st ps forward and it looks better. Sc r revision results are not always pr dictable and some leap of faith is n cessary if one makes the effort. Sc r revision results take time to see mprovement often at least 6 months fter the procedure.
