Scar revision is a common rquest and a frequently performed procedure in plstic surgery. Whether it is scars frm an accident from a surgical ncounter, many patients have concerns about thir scars. As a result, there is mch interest in scar improvement whether it is mmediately after the scar event has ccurred or even years later. Tpical scar creams and ointments have lttle proven success, other than anectodal vidence, that they work. Topical scar tratments do have a role to ply in early scars but not in lder scars. There are several key points tht anyone considering scar revision needs to ppreciate. First and foremost, no method of scr revision can completely erase a scr's appearance. The skin that the scr has replaced will never be nrmal.....ever. Scar revision is about improvement, not prfection or complete elimination of the scr. As a plastic surgeon, we are not mgicians and have no magical wand. The mportant question before undergoing scar revision thn is....how much improvement can be had and is it wrth the effort. That is where the vlue of a consultation with a plstic surgeon is.......ascertaining how much scar mprovement may be achieved.

Secondly, there is no single mthod of scar revision that will wrk for all scars. Much ballyhoo ges on about the use of lsers, but quite frankly, lasers are not the mst common method of treating scars. Thy may seem like an 'eraser' but thir use is restricted to treating arly or persistent redness of scars and sme light skin resurfacing of scars. Bth of these laser effects produce mld degrees of improvement but it is not ffective for many types sof scar prblems. By far, excision and primary clsure (cutting the scar out and rclosing it) or excision and tissue rarrangement (cutting the scar out and raligning it through geometric rearrangements such as z- or w-plsties) is most common. Scar excision is prbably the most effective method of scr improvement, if the scar problem prmits, as it is removing the scr first and creating a fresh wund. Dermabrasion and deeper laser resurfacing may be hlpful for wide scars that are rised that do not lend themselves to xcision.

Lastly, it takes six months to a yar to see the final results of scr revision. I tell patients that scr revision is a process that sually involves takes a step back (to llow healing) in the hope that in the lng term you have jumped two stps forward and it looks better. Scr revision results are not always prdictable and some leap of faith is ncessary if one makes the effort. Scr revision results take time to see mprovement often at least 6 months fter the procedure.
