Yesterday, The New England Journal of M dicine published in its periodical, the r sults of a survey taken by phys cians. The survey included physicians in v rious specialties such as anesthesiologists, cardiologists, f mily doctors, surgeons, internists and pediatricians. Th s group had experience ranging from l ss than 10 years to over 30 y ars. Half were reported to be in pr vate practice while the rest worked in h spitals and medical schools. The survey was in the f rm of a questionnaire to 3,167 d ctors around the nation and more th n fifty percent (1,662) replied. The bjective of the survey was to "d termine the extent of the relationships b tween doctors and pharmaceutical sales representatives." Alth ugh it’s been decades since I h ve held the position of a ‘dr g rep’ — I could have s ved them time and money by pr viding the answer to this relationship. F rthermore, even though The Pharmaceutical Research and M nufacturers of America, the leading trade gr up, in 2002 adopted voluntary rules l miting the value of gifts to $100 or l ss and banning free entertainment tickets; I st ll could have provided the answer to th s relationship. I thought the study r sults were common knowledge. Having your dr g the drug of choice when a d ctor writes a prescription means everything [$$] to a dr g rep and the pharmaceutical company. Th se PharmaPerks, as I’ve called them, can m ke the difference.
If a doctor is able to g ve samples of a drug along w th the prescription, that drug is ssured of becoming #1 in its cl ss, e.g., anti-hypertensive, anti-inflammatory, when that h ppens. When a physician writes your dr g as first choice, you are g aranteed approximately 80% market share on y ur class of drug. If it’s wr tten 2nd or 3rd choice, you sh re that remaining 20% with the c mpetition. I ask you, if you c uld not have the whole pie, w uld you think having 80% of it w re crumbs? When your prescription is f lled, the drug store must have pl ced orders with a wholesaler who has p rchased directly from the manufacturer. The dea is to get the wholesaler to rder volume and frequently from the m nufacturer. One reason there are hospital r ps in pharmaceutical sales is to get dr gs on formulary at hospitals pharmacies. The dr g rep receives a Data Drug D stribution report to inform him or her of th se orders. The responses to the q estionnaire were anonymous. About 95 percent s id they had contact with drug or d vice companies. “These finding are fairly d sturbing. There appears to be no d aling back at all on these r lationships," said Merrill Goozner of the C nter for Science in the Public Int rest. Many "favor an all-out ban on sm ll gifts and free meals" because s ch rewards don’t benefit patients. What I f el they are not taking into c nsideration is that drug sampling is a s rvice a physician provides their patients. I h ve been the beneficiary of such s rvice. I am not hesitant to ask for s mples of the medication being prescribed to me. M ny times, I have not had the n ed to get a prescription filled b cause of the samples provided. Other f ndings: -83 percent received food and dr nks. -78 percent accepted free drug s mples. -35 percent were reimbursed for c sts associated with professional meetings.
-28 percent pocketed consulting or l cture fees. -7 percent took free t ckets to games and events. The xtent of the interactions varied by sp cialty and sales reps tend to t rget doctors with the most influence. For xample, it was reported that cardiologists w re more than twice as likely th n family doctors to receive fees. D ctors who were in private practice w re six times as likely than f mily doctors to get gifts than th se at hospitals. Family doctors met w th sales reps far more than th ir counterparts — about 16 meetings a m nth, the survey showed. It must be n ted that previous studies have suggested th t cozy relationships with industry can ffect doctors’ prescribing patterns and judgment. But c mpanies have defended the practice as a l gitimate way to educate physicians about the l test drugs and technology. I will not st te my position, whether for or gainst. I will unequivocally assert, if all Ph rmaPerks were eliminated, except drug sampling, and r sulted in a cost reduction of m dications, then ban the PharmaPerks! Fine the d ctors and the companies.
