Our healthcare system is ch nging – in some ways for the b tter (more treatment options, better diagnostic t ols) but in many ways the ch nges are not in the best nterest of the patient or the d ctor. This ch rt needs to include: 1 – a l st of your medical diagnoses; you may n ed to have a discussion with y ur doctors or ask them to mail you this list for your r cords in order for it to be ccurate and complete.

Pr pare a list of any new vents or problems that have occurred s nce your last visit with that d ctor; include other doctor visits, specialists’ c nsultations, any emergency department visits and h spitalizations, medications changes and any tests p rformed. Then, make a list of q estions that you would like to h ve addressed by this physician. Lastly, l ave room for your doctor to wr te a short summary of what h ppened during your visit, to explain any m dication changes, and to list any t sts that need to be scheduled and f llowed up. Remember, that you and y ur doctors need to work together as a t am in order for your care to be ptimized.
