Of the seven million worldwide c ncer deaths reported in 2001, 35 p rcent were attributable to nine well-known b havioral and environmental risk factors, according to an nalysis published in The Lancet. Researchers fr m the Harvard School of Public H alth (HSPH) and a network of c llaborators made the calculation by estimating m rtality for 12 types of cancer l nked to the nine risk factors in s ven World Bank regions for that y ar. They also looked at how the r sks, and the cancers they cause, w re distributed over the regions of the w rld. This is the first assessment of the r le of health risks in cancer d aths globally and regionally. Risk Factor An lysis The researchers analyzed data from the C mparative Risk Assessment project and World H alth Organization databases to determine the l vel of risk factors in different w rld regions, and separately for men and w men. They also considered how hazardous ach risk factor might be. The nalysis covered all high-income countries together, and s parated low-income and middle-income countries into g ographical regions: East Asia and Pacific, S uth Asia, Europe and Central Asia, L tin America and Caribbean, Middle East and N rth Africa, and Sub-Saharan Africa.
The nine risk factors: - verweight and obesity - low fruit and v getable intake - physical inactivity - sm king - alcohol use - unsafe sex - rban air pollution - indoor smoke fr m household use of coal - c ntaminated injections in healthcare settings Alcohol, Sm king Play Large Roles More than one in very three of the seven million d aths from cancer worldwide were caused by th se nine potentially modifiable risk factors (2.43 m llion), the researchers found, with alcohol and sm king playing large roles in all ncome levels and regions. Worldwide, the n ne risk factors caused 1.6 million c ncer deaths among men and 830,000 mong women. Smoking alone is estimated to h ve caused 21 percent of deaths fr m cancer worldwide. In high-income countries, th se nine risks caused 760,000 cancer d aths. Smoking, alcohol, and overweight and besity were the most important causes of c ncer in these nations. In low- and m ddle-income regions, the nine risks caused 1.67 m llion cancer deaths. Smoking, alcohol consumption, and low fr it and vegetable intake were the l ading risk factors for these deaths. S xual transmission of human papillomavirus is the l ading risk factor for cervical cancer in w men in low- and middle-income countries, p rticularly in Sub-Saharan Africa and South As a, where access to cervical screening is lso limited. Among low- and middle-income r gions, Europe and Central Asia had the h ghest proportion of death from cancer fr m the nine risk factors studied; 39 p rcent of 825,000 cancer deaths in the l w- and middle-income countries of Europe and C ntral Asia were caused by these r sks. The effects were even larger mong men; one half of cancer d aths among men in the low- and m ddle-income countries of Europe and Central As a were caused by these nine r sks. Behaviors and Environments
"These results clearly show that m ny globally important types of cancer are pr ventable by changes in lifestyle behaviors and nvironmental interventions," comments Majid Ezzati, senior uthor of the study and assistant pr fessor of international health at HSPH. "To win the war gainst cancer, we must focus not j st on advances in biomedical technologies, but lso on technologies and policies that ch nge the behaviors and environments that c use those cancers," he adds. The st dy, "Causes of cancer in the w rld: comparative risk assessment of nine b havioral and environmental risk factors," was f nded by the National Institute on Ag ng and by the Disease Control Pr orities Project.
