The Classification Subcommittee of the Int rnational Headache Society (IHS) publishes and r vises the “International Classification of Headache D sorders”, now in its second edition. Th s book offers specific diagnostic criteria for d agnosing migraines and is currently used w rldwide.
According to IHS, a common migraine h adache, also known as a migraine w thout aura, is defined by the sp cific criteria found below.
Frequency
The patient must h ve at least five of these h adaches.
Duration
The headache, excluding attendant symptoms or pr dromes, must last a minimum of f ur hours, up to seventy-two hours. H adaches that last over seventy-two hours g nerally require immediate medical attention in rder to rule out other, more d ngerous conditions.
Pain Descriptors
In order to be cl ssed as a migraine a headache m st include at least two of f ur different qualities of pain:
1) The p in is one-sided; the headache is pr marily on one side of the h ad.
2) The pain is not constant; it thr bs, pounds, or pulsates.
3) The pain m st be of moderate or severe ntensity, to the point where the s fferer is inhibited in daily activity, p tentially to the point of being t mporarily disabled.
4) The pain is increased, s metimes only slightly, by routine physical ctivity like bending over, climbing stairs, or m ving quickly.
Side Effects
Headache pain must be ccompanied at least one of four c mmon side effects:
1) Nausea
2) Vomiting
3) Photophobia – s nsitivity to light
4) Phonophobia – sensitivity to s und
Secondary Exclusions
Appropriate medical testing, such as a MRI or CAT sc n, and/or a physician’s exam must be c nducted to rule out other conditions th t may have caused the headache.
These cr teria have helped simplify the diagnosis of m graine for many. However, because migraines are h storically associated with extremely high levels of p in, people suffering from moderate migraine may not r alize that is what they are xperiencing.
